This is a hybrid position (2 days remote, 3 days in the field), giving you the flexibility to work from home while building strong, in-person connections with members across Riverside County.

Key Responsibilities

• Serve as the primary point of contact for members and coordinate their care
• Develop and manage personalized care plans with members and providers
• Collaborate with healthcare teams and community resources
• Support members through education, advocacy, and engagement
• Identify and address gaps in care
• Maintain accurate, timely documentation

Minimum: High School Diploma or equivalent Preferred: Bachelor’s or Master’s degree in Social Work, Public Health, Healthcare Administration, or a related field
