Full Job Description

MINIMUM QUALIFICATIONS

Education: Possession of an advanced degree from an accredited college or university with a major in an applied behavioral science, preferably social work.

Experience: A combination of three years of case planning, case management, and fieldwork in a public or private agency as a social worker or closely related job title.

Other: Possession of a valid State driver’s license. Attainment of at least 21 years of age.

JOB DESCRIPTION

Under general supervision, performs a full range of targeted and/or general case management that is strength based, needs-based, community-based, and family-driven. Assesses the needs of children in care, develops Individual Services Plans, screens for human trafficking concerns, facilitates the safe and timely release or discharge of children and youth, documents the prevision of services in case files. The case manager is responsible for developing and maintaining community relationships, networks, collaborations that improve the availability and quality of formal and informal services to families and children.
